A 38-year-old male presented with an extremely rare primary tumor of the pineal region with papillary features manifesting as headaches, memory impairment and gait disturbance, persisting for one month. Magnetic resonance imaging showed a well-defined mass, with partial enhancing patterns, in the region of the pineal gland. The tumor was biopsied for pathologic confirmation after an endoscopic third ventriculostomy. The histological diagnosis was a papillary tumor of the pineal region (PTPR), which has been recently described as a distinct clinicopathological entity. Postoperatively, the patient underwent adjuvant radiation therapy and continued to do well, with no recurrence at the 23 month follow-up examination. PTPR should be considered in the differential diagnosis of other pineal tumors because misdiagnosis often occurred. This is still a rare tumor, so the full spectrum of its biological features, natural clinical course, optimal treatment modalities and its response to adjuvant chemotherapy or radiotherapy have yet to be determined.
